We wish the members details to be made available to other CFW members, to CFW’s professional advisers, to commissioners and to members of the public, for instance through CFW’s website.
On behalf of the Applicant, I agree to the following conditions
of membership of Care Forum Wales (“CFW”):
We shall at all times follow, uphold and promote the principles and spirit
of the Dignity in Care Charter.
All care homes / care businesses in Wales registered with CSSIW that are linked with the Applicant [are already] [are now applying to become] CFW members
[Or: there are no other care homes / care businesses in
Wales that are linked to the Applicant]
The minimum period of membership of CFW is 12 months.
We shall give three months’ written notice if we wish to end our membership of CFW.
The CFW logo is a registered trademark and remains the property of CFW at all times. We may display or use the logo only for the purpose of promoting our organisation’s care services and only so long as we are current, paid members of CFW.
CFW may terminate our membership (whether on notice or immediately, depending upon the circumstances) in the event of any breach of these conditions or any conduct by me / us that might bring CFW into disrepute. [An appeal against termination is available – please ask for details.]
If our membership of CFW comes to an end for any reason, we shall immediately cease to use or display the CFW logo and shall, within three working days, at our own expense and by ‘special delivery’ post, return to the address below any CFW plaque we possess.
Please delete / amend as appropriate. A ‘linked’ care organisation means another home or agency operated by the same provider or by a related provider e.g. a group of companies or partnerships.
